What is Adjusted Body Weight (AjBW)?

Adjusted Body Weight is a measure used by healthcare professionals to determine medication dosages (such as aminoglycosides or heparin) and nutritional requirements in patients who are overweight or obese. It provides a middle ground between Actual Body Weight and Ideal Body Weight.

The Formulas Used

Our calculator first determines your Ideal Body Weight (IBW) using the standard Devine Formula:

  • Males: 50 kg + 2.3 kg for each inch over 5 feet.
  • Females: 45.5 kg + 2.3 kg for each inch over 5 feet.

Then, the Adjusted Body Weight is calculated as:

AjBW = IBW + 0.4 × (Actual Weight - IBW)

When to Use Adjusted Body Weight

Clinicians typically use Adjusted Body Weight when a patient's actual weight is more than 20% to 30% above their ideal body weight. In these cases, using actual weight might lead to over-dosing, while using ideal weight might lead to under-dosing.
